Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager to coordinate in-home and community-based care for dually-eligible enrollees with complex needs. You will monitor, follow up, and manage care plans in collaboration with an interdisciplinary team, optimizing outcomes and quality of life.

The role requires an RN with strong care coordination experience, knowledge of Medicare/Medicaid, and the ability to travel to member homes and community settings. A state license is required.

Qualifications

• Associates of Science (A.S) degree in nursing required.
• Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs.
• Prior care coordination/case management experience preferred.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Engage enrollee in homes and community settings to establish complex care management relationships.
• Lead interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with providers and community resources to ensure seamless communication and transitions.
• Oversee enrollee utilization of long-term services and supports to keep them in their chosen location.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
