Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager (RN) to lead complex care management for dually-eligible enrollees in community settings. You will coordinate health services, collaborate with the interdisciplinary care team, and guide care plans to improve outcomes and quality of life, with travel to member homes and provider sites.

Qualifications

• Must be a licensed RN in Massachusetts.
• Associate degree in Nursing required.
• Experience in care coordination for dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Lead the ICT to develop holistic care plans.
• Coordinate with healthcare providers and community resources.
• Perform comprehensive assessments and risk assessments.
• Support enrollee transitions and avoid readmissions.
• Educate enrollees and families on benefits and preventive care.
• Travel to member homes and provider sites.
