Job summary

CareSource in Massachusetts is seeking an Integrated Care Clinical Manager (RN) to lead complex care management for dually-eligible enrollees, coordinating health and community resources to improve outcomes and quality of life.

This field-based role requires regular travel, leadership of the ICT, and collaboration with healthcare providers to create holistic care plans and facilitate seamless transitions across settings.

Qualifications

• ASN in Nursing from an accredited program.
• Active MA RN license; ability to independently care for complex cases.
• Case Management Certification preferred.
• Experience with dual-eligible populations and managed care is preferred.

Responsibilities

• Engage enrollees in homes and community settings to build complex care management relationships.
• Coordinate care as liaison between providers, resources and enrollees.
• Perform comprehensive assessments and risk evaluations as required.
• Lead the interdisciplinary care team to develop holistic care plans.
• Monitor long-term services and supports and ensure appropriate settings for enrollees.
• Assist members in accessing housing, transportation, and social services.
• Educate members about Medicare/Medicaid benefits and preventive care.
• Travel regularly to member and provider sites; ensure continuity of care.
