Job summary

CareSource is seeking an Integrated Care Clinical Manager (RN) to lead community-based care management for dually-eligible enrollees in Massachusetts. The role focuses on coordinating medical, behavioral, and social services to improve outcomes and quality of life.

Responsibilities include coordinating with healthcare providers, community resources, and family education, while guiding the interdisciplinary team to develop comprehensive care plans. Travel across MA is required.

Qualifications

• Associates of Science in Nursing from an accredited program.
• Current unrestricted MA RN license.
• Experience in care coordination or case management.
• Medicare/Medicaid managed care knowledge preferred.
• Must be able to travel as required and drive.

Responsibilities

• Engage with enrollees in homes and communities to manage complex care.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources to avoid duplication of services.
• Educate enrollees about benefits, medications, and preventive care.
• Follow up after health events to ensure continuity of care and prevent readmissions.
