Job summary

Qualifications

• Associates of Science in Nursing from an accredited program.
• Current RN licensure in Massachusetts.
• Experience in care coordination or case management with dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in home and community settings to build complex care management relationships.
• Coordinate with healthcare providers and community resources for seamless care transitions.
• Perform comprehensive assessments and functional evaluations as needed.
• Develop and implement care plans with ongoing updates based on enrollee status.
• Lead the interdisciplinary care team to address medical and non-medical needs.
• Ensure access to long-term services and supports and prevent readmissions.
• Educate members and families about benefits, chronic disease management, and preventive care.
