Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts. This RN-led role focuses on monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs.

You will coordinate care across providers, community resources, and social services to improve outcomes and quality of life. The position requires an active RN license in Massachusetts, care coordination experience, and willingness to travel up to 50% to member, provider, and community visits.

Qualifications

• AS degree in nursing from an accredited program.
• Current unrestricted RN license in Massachusetts.
• Experience in care coordination, case management, or dual-eligible populations preferred.
• Case Management Certification is highly preferred.

Responsibilities

• Engage enrollees in their homes and community settings to establish complex care management.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate care with providers and community resources to avoid duplication of services.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
