Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts. The role is a community-based, RN-led position focusing on monitoring, follow-up, and clinical care management for dually eligible enrollees with complex medical, behavioral, and social needs.

The manager will lead an interdisciplinary team, coordinate with healthcare providers, and ensure effective care transitions, with travel across communities to support members in living in their preferred setting.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required.
• Current unrestricted clinical license in Massachusetts as an RN required.
• Case Management Certification preferred.

Responsibilities

• Engage with enrollees in homes and community settings for complex care management.
• Lead interdisciplinary care team to create holistic care plans.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Monitor enrollee utilization of long-term services and supports.
