Job summary

CareSource is seeking an Integrated Care Clinical Manager (Massachusetts) to oversee complex care management for dually-eligible enrollees. As an RN, you will lead an interdisciplinary team, conduct comprehensive assessments, and coordinate with primary care providers and community resources to optimize health outcomes.

Qualifications

• Associates of Science (A.S) degree in nursing required.
• RN license in Massachusetts required.
• Experience in care coordination or with dual-eligible populations preferred.
• Medicare and Medicaid knowledge preferred.

Responsibilities

• Engage with enrollees in homes and community settings for complex care management.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate with healthcare providers and community resources to ensure smooth care transitions.
• Monitor enrollee utilization of long-term services and supports.
• Educate enrollees and families about benefits, chronic condition management and preventive care.
• Travel to member, provider and community visits as required.
