Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts. This RN will monitor, follow up, and manage complex cases for dually-eligible enrollees, coordinating with providers and community resources to improve health outcomes and quality of life.

Key duties include leading the ICT, assessing needs, coordinating long-term supports, and educating members on Medicare/Medicaid benefits. Occasional travel is required within the Commonwealth.

Qualifications

• Associate of Science (A.S) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs.
• Prior experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Provide in-home and community-based care management for dually-eligible enrollees with complex needs.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Educate enrollees and families about benefits, preventive care, and self-management.
