VOA Illinois is seeking an Intake Service Coordinator to serve as the central access point for clients, manage eligibility and enrollment, and coordinate seamless transitions into ongoing case management. You will work with internal teams and partners to ensure timely access to services.

The role emphasizes trauma-informed engagement, documentation accuracy in EV and HMIS, and advocacy to reduce barriers to service access for veterans and families in Chicago.

Qualifications

• Illinois MHP requirements may include degrees or licensure.
• 5 years of supervised mental health or human services experience.
• Certifications include PRP, Recovery Support, or OT Assistant with applicable experience

Responsibilities

• Serve as primary contact for potential clients seeking services.
• Conduct intake assessments and eligibility screenings.
• Assist with service planning, resource coordination, and stabilization.
• Educate clients on VOA Illinois programs and services
