Job summary

The Salvation Army North and Central Illinois Division is seeking an SSVF Health Care Navigator Coordinator to assist Veterans in gaining access to health care through VA or community providers. The role focuses on removing barriers to care, education on wellness, and supporting enrollment processes with VA and community health systems.

The position is full-time, exempt, and located in Decatur/Mattoon IL region.

Qualifications

• Bachelor's degree required with relevant field.
• Experience in social services, healthcare, or homeless programs preferred.
• Valid driver’s license required.

Responsibilities

• Perform specialized case management and care coordination with veterans to access VA or community healthcare.
• Enter and maintain case data in HMIS/SIMS with accuracy.
• Collaborate with health care teams, VA facilities, and community partners to facilitate enrollment and appointments.
