Job summary

VSHP Volunteer State Health Plan, Inc. in Sumner County is seeking a dedicated Support Coordinator to help individuals with intellectual and developmental disabilities live independently and find integrated employment.

You will assess needs, develop plans, and coordinate home and community-based services with a multidisciplinary team. This role requires a valid driver's license, willingness to travel to members' homes, and strong communication, organization, and Microsoft Office skills.

Qualifications

• RN, with CDDS or DDC preferred for RNs, or BS in social work, nursing, education, human service or health care field.
• Licensed LMHC/LSW preferred; or meet QDDP/QIDP requirements per federal standards.
• 5+ years experience as an independent Support Coordinator or HCBS waiver case manager.

Responsibilities

• Partner with members and families to identify supports and services to reach health, employment, and independent living goals.
• Collaborate with clinical and social support staff to meet health and long-term service needs.
• Conduct face-to-face visits to assess status and needs, including physical, behavioral, functional, and employment expectations.
• Authorize appropriate home and community-based services and verify they are provided and meet needs.
