Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a Support Coordinator to empower individuals with intellectual and developmental disabilities in Davidson County.

You will coordinate health, behavioral, and social supports to help members achieve independence and secure integrated employment through person-centered planning. You will conduct intake, enrollment, and ongoing assessments, collaborate with clinical teams, and ensure services meet member needs.

Qualifications

• RN with current certification or related nursing credential is preferred.
• Or B.S. in social work, nursing, education, human services or health care field; LMSW or LCSW preferred.
• Or meet federal requirements for QDDP or QIDP.
• In lieu of a degree, 5+ years of experience as a Support Coordinator or case manager for HCBS Waiver.

Responsibilities

• Partner with members and families to identify supports and services for health, employment, and independent living.
• Collaborate with clinical and social support colleagues to meet health and long-term service needs.
• Conduct face-to-face visits to assess current status and needs across physical, behavioral, functional, psycho-social, and employment dimensions.
• Authorize appropriate home and community based services and verify ongoing provision.
• Maintain valid driver’s license.
• Immunizations or medical tests may be required.
• Digital literacy assessment is required.
