Job summary

VSHP Volunteer State Health Plan, Inc is seeking a Support Coordinator to empower individuals with I/DD across Cumberland, Roane, Morgan and Blount County. The role involves intake, enrollment, home visits, and coordinating in-home and community services to promote health, independence and employment.

Travel to members’ homes is required with mileage reimbursement. The ideal candidate holds RN with relevant DD certification or a QDDP/QIDP, or a B.S.

Qualifications

• Education and/or credentials as RN with relevant DD certification or QDDP/QIDP.

Responsibilities

• Partner with members and families to identify supports and services to meet goals for health, employment and independence.
• Conduct face-to-face visits to assess physical, behavioral, functional, psycho-social, financial and living needs.
• Coordinate and authorize Home and Community Based Services per eligibility and plan.
• Maintain ongoing assessments and update Person-Centered Plans.
