Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a Support Coordinator to empower individuals with I/DD in Maury/Williamson County, TN. You will assess needs, coordinate home and community-based services, and collaborate with clinical teams to support health, behavior, and independence.

The role requires a valid driver’s license, and travel to members’ homes is reimbursed. Candidates with RN or related licensure and relevant certifications are encouraged to apply.

Qualifications

• RN with preferred certification from DDNA as CDDN for RNs or DDC for LPNs
• BS in social work/nursing/education or related field; LMSW/LCSW preferred
• QDDP/QIDP federal requirements or equivalent experience
• In lieu of degree, 5+ years independent Support Coordinator/case management in 1915(c) HCBS Waiver and completed workshops

Responsibilities

• Partner with members and families to identify needed supports and direct services
• Collaborate with clinical/social support team to meet health and long-term service needs
• Conduct face-to-face visits to assess current status and needs across multiple domains
• Authorize home and community-based services as appropriate and verify delivery
• Maintain driver license and comply with any related safety programs
