Job summary

BlueCross BlueShield of Tennessee, Inc. is seeking a dedicated Support Coordinator to empower individuals with I/DD in Sumner County. You will collaborate with members and families to identify supports, arrange services, and support health, employment, and independent living goals.

The role includes intake, enrollment, assessments, and team collaboration, plus travel to members’ homes. A valid driver’s license and digital literacy are required; mileage is reimbursed.

Qualifications

• RN with current license and CDDS/DDC or QDDP/QIDP eligibility per federal requirements.
• BS in social work, nursing, education, human services or related field; MSW/LCSW preferred.
• 5+ years as independent Support Coordinator or case manager in 1915(c) HCBS Waiver, with Personal Outcome Measures training.
• Healthcare/operations experience required.

Responsibilities

• Partner with members and families to identify needs and direct services for good health, employment, and independent living.
• Collaborate with clinical and social support teams to meet health and long-term service needs.
• Conduct thorough face-to-face visits and assess member status across physical, behavioral, functional, psycho-social, financial, and employment dimensions.
• Authorize appropriate home and community-based services and ensure needs are met.
• Maintain valid driver’s license and comply with safety and training requirements.
