Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a Support Coordinator to empower individuals with I/DD in Maury/Williamson County, TN. You will assess needs, plan services, and coordinate care across health, behavioral, and community supports.

The role requires an RN or related health-care degree (QDDP/QIDP eligible preferred) and 2+ years in health care. A valid driver's license is required and mileage is reimbursed.

Qualifications

• RN with related health care degree or licensure; QDDP/QIDP eligibility preferred.
• 2+ years in healthcare/operations or related field.
• Proficient in Microsoft Office (Outlook, Word, Excel, PowerPoint).
• Ability to work independently or in a team; strong communication.
• Must have valid driver’s license and willingness to travel within counties.

Responsibilities

• Partner with members and families to identify supports for health, employment, and independent living.
• Collaborate with clinical and social support teams to meet needs.
• Conduct face-to-face visits to assess status and needs across multiple domains.
• Authorize home and community based services and ensure delivery.
• Ensure mileage is tracked and services meet member goals.
