Job summary

VSHP Volunteer State Health Plan, Inc. seeks a Support Coordinator to empower members and coordinate health, behavioral, and social services in Sumner County.

You will conduct intake, assessments, and develop Person-Centered Plans to support independent living and community employment. The role requires clinical or social work background, 2+ years in healthcare or operations, and strong communication and organizational skills.

Qualifications

• RN qualifications or related degree with applicable licensure
• Experience in healthcare or operations environment (2+ years)
• Certifications or credentials for developmental disabilities preferred or QDDP/QIDP eligibility

Responsibilities

• Partner with members and families to identify needed supports and direct services
• Collaborate with clinical and social support team to meet health and long-term service needs
• Conduct thorough face-to-face visits to assess needs across physical, behavioral, functional, psycho-social, financial and employment domains
• Authorize appropriate home and community-based services and ensure needs are met
