Job summary

VSHP Volunteer State Health Plan, Inc in Tennessee is seeking a dedicated Support Coordinator to join ECF Choices, serving Monroe, McMinn and Polk County. You will partner with members and families to identify supports, help them access services, and promote health, employment and independent living.

The role involves intake and enrollment, coordinating with clinical and social support colleagues, conducting visits, and ensuring services meet needs.

Qualifications

• RN with CDDS, or DDC/DDN for RNs as applicable.
• BS in social work, nursing, education, human services or health care.
• QDDP/QIDP certifications preferred.
• 5+ years as independent Support Coordinator or case manager with HCBS waiver trainings.

Responsibilities

• Partner with members and families to identify supports and services.
• Collaborate with clinical and social support colleagues to meet needs.
• Conduct face-to-face visits to assess health, behavior and living needs.
• Authorize and monitor home and community based services.
• Maintain a valid driver’s license and travel to homes; mileage reimbursed.
• Use Personal Outcome Measures introductions and assessment workshops when applicable.
