Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a dedicated Support Coordinator to empower individuals with I/DD in Monroe, McMinn and Polk County, TN. You will conduct home visits, assess needs across physical, behavioral, and social domains, and develop Person-Centered Plans to promote independence and employment.

Ideal candidates hold RN with appropriate DD certifications or a related degree, plus 5+ years of HCBS experience. Travel to member homes is required; mileage is reimbursed.

Qualifications

• RN with current certification as CDNN/CDD or related; QDDP/QIDP eligible.
• BS in social work, nursing, education, health care; licensure preferred.
• Qualified Developmental Disabilities Professional (QDDP) or QIDP per federal rules.
• 5+ years’ experience as a Support Coordinator in 1915(c) HCBS Waiver environment.

Responsibilities

• Partner with members and families to identify supports and direct services.
• Collaborate with clinical and social support colleagues to meet health and long-term needs.
• Conduct face-to-face visits and assess physical, behavioral, functional, psycho-social and employment needs.
• Authorize home and community-based services and ensure services are provided.
• Maintain driving to member homes and mileage reimbursement where applicable.
