Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a dedicated Support Coordinator to empower members with I/DD to live independently and secure integrated employment.

You will perform intake, enrollment, and ongoing assessments, develop person-centered plans, and coordinate team-based supports across health, behavioral, and social services in Monroe, McMinn, and Polk County. Ideal candidates will have RN with related certifications or a social work/healthcare background, plus 2+ years in

Qualifications

• RN/CDDN or DD Nurse preferred; or social work/healthcare degree or related field.
• LMSW/LCSW preferred; or QDDP/QIDP or 5+ years experience in HCBS Waiver.

Responsibilities

• Partner with members and families to identify supports and direct services.
• Collaborate with clinical and social support team to meet health and long-term needs.
• Conduct thorough face-to-face visits to assess status and needs.
• Authorize appropriate home and community-based services.
• Ensure services are provided and needs are met.
• Maintain driver license and vehicle policies as required.
