Job summary

VSHP Volunteer State Health Plan, Inc is seeking a dedicated Support Coordinator to empower individuals in the Cumberland, Roane, Morgan, and Blount County areas. You will coordinate health, behavioral, and social supports, visit members in person, and help them achieve independence and employment in their communities.

The role requires RN with applicable certifications or a related bachelor’s degree, plus 2+ years in healthcare operations.

Qualifications

• RN with current certification as CDDS/CL or DDN/DDC preferred.
• BS in social work, nursing, education or related field; licensure preferred.
• QDDP/QIDP eligibility or related credentials acceptable.
• 5+ years as an independent Support Coordinator or case manager.

Responsibilities

• Partner with members and families to identify supports and services.
• Collaborate with clinical and social teams to meet health needs.
• Conduct face-to-face visits assessing physical, behavioral, functional needs.
• Authorize home and community based services per criteria.
• Maintain driver’s license and comply with safety policies.
• Complete immunizations/tests and digital literacy assessments.
