Job summary

Qualifications

• Certification by ASHA (CCC-SLP or CCC-A)
• Minimum six months experience as speech-language pathologist
• Current and unrestricted license
• CPR certification required
• Driver’s license and reliable transportation required

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility
• Participate in plan of treatment and plan of care development
• Assess and treat disorders of speech, language, voice, hearing and swallowing
• Administer and interpret diagnostic tests and apply therapeutic treatments
• Observe and report changes in patient condition to supervisor/physician
• Provide instruction on alternative communication systems when appropriate
• Counsel patients, families and healthcare staff
• Submit documentation and progress reports per policy
• Participate in care coordination and discharge planning
