Job summary

CenterWell Home Health seeks a Speech Language Pathologist to provide in-home therapy, evaluate and treat patients to improve communication and swallowing, and support rehabilitation in the home setting.

You will develop and review plans of care, document progress, and coordinate with the clinical team while ensuring licensure and safety compliance. A valid state license, ASHA CCC-SLP, CPR, and a valid driver’s license are required. This is a full-time role with 40 scheduled hours per week.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including auditory screening.
• Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or physician.
• Provide instruction and training in alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff.
• Maintain and submit documentation and reports as required by the Company.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality of patient information in line with policy.
