Job summary

CenterWell Home Health, part of Humana Inc., is seeking a Speech Language Pathologist to deliver in-home therapy to patients. You will evaluate, treat and coordinate care to improve communication, swallowing and quality of life.

You will work under the Clinical Manager and participate in care planning, documentation and family education. The role requires six months of experience, an active SLP license, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize effects.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction and training in alternative communication systems as appropriate.
• Maintain and submit documentation and written reports as required by policy.
• Participate in care coordination activities and discharge planning.
