Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to evaluate and treat patients in home settings, collaborating with the care team to develop plans of treatment and care. You will assess speech, language, voice, swallowing and cognitive communication disorders, provide therapy, and document progress while supporting patients and families.

Requirements include ASHA CCC, current licensure, CPR, a valid driver’s license and home health experience preferred.

Qualifications

• CCC or equivalent ASHA certification required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted professional license.
• CPR certification on file.
• Valid driver's license, insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility.
• Develop and review Plan of Treatment and Care with the team.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and assessments.
• Document changes and report to supervisor/physician.
• Provide patient, family and staff training and counseling.
• Maintain timely documentation and care coordination.
