Job summary

CenterWell Home Health, a Humana company, is seeking a Speech Language Pathologist to deliver in-home or facility-based services to patients. You will evaluate, treat, and coordinate care to improve communication, swallowing, voice, and cognitive function, while documenting progress and updating plans of care.

Requirements include ASHA CCC-SLP or eligibility, a current license, CPR, and a valid driver's license with reliable transportation; home health experience is preferred.

Qualifications

• ASHA CCC-SLP certification required.
• Minimum six months experience as a speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation required.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction to patients/families on communication aids and strategies.
• Maintain documentation and participate in care coordination and discharge planning.
