Job summary

CenterWell Home Health is seeking a licensed Speech Language Pathologist to deliver in-home assessments, therapy, and care plans. You will evaluate, treat, and document progress for patients in their homes or facilities, collaborating with the Clinical Manager and care team.

You should have ASHA CCC-SLP or CCC-A, current license, CPR, and be able to drive. Pay per visit ranges from $54 to $76, with full-time hours and Humana benefits.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA)
• Minimum six months experience as a speech-language pathologist
• Current and unrestricted license
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate, direct and provide speech-language pathology services in home or facility
• Develop and review Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and therapeutic applications
• Record changes in patient condition and report to supervisor/physician
• Provide training on alternative communication systems when appropriate
• Convey information to patients, families, and healthcare staff
• Maintain and submit required documentation and care plans
• Participate in care coordination and discharge planning
• Maintain confidentiality and professional conduct
