Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to provide in-home or facility-based therapy, evaluating and treating speech, language, voice and swallowing disorders. You will participate in plans of care and coordinate with the care team to support patient independence.

The role requires a current SLP license, CPR certification, and a valid driver’s license with reliable transportation. Experience in home health is preferred and compensation reflects market rates for full-time

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize effects.
