Job summary

CenterWell Home Health, a Humana company, seeks a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities, developing and reviewing treatment plans.

You will deliver therapy for speech, voice, language and swallowing disorders, educate patients and families, and document progress in compliance with company policies.

Candidate must hold ASHA CCC, current license, CPR, and a valid driver’s license with reliable transportation to serve home health patients.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Assess, treat and manage disorders of speech, voice, language, hearing and swallowing
• Observe, record and report changes in patient condition to supervisor/physician
• Provide instruction to patients and families in alternative communication systems
