Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and provide speech-language pathology services in patients’ homes or facilities. You will participate in care planning, assess disorders, and support patients and families with communication strategies.

The role requires ASHA CCC-SLP, a valid license, CPR certification, and reliable transportation. Travel and patient-facing duties are involved as part of Humana's CenterWell team.

Qualifications

• Must meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist/speech language pathologist.
• Current and unrestricted license; CPR certification; valid driver's license and transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests including audiologic screening.
• Document changes and progress; report to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation.
• Submit reports and participate in care coordination and discharge planning.
• Maintain confidentiality and appropriate information sharing.
