Job summary

Humana Inc's CenterWell Home Health is seeking a licensed Speech Language Pathologist to deliver in-home therapy. You will evaluate, treat, and direct SLP services in patients' homes or facilities, developing plans of care and coordinating with care teams.

The role requires at least six months of SLP experience, a valid license, CPR, and a valid driver's license with reliable transportation. Pay ranges from $102,500 to $141,000 per year, with 40 hours weekly; benefits are provided.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient's condition and response to treatment.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility.
• Prepare and submit written reports of evaluations, visits, summaries, care plans and progress reports.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality and share information only when appropriate.
