Job summary

Providence at Home with Compassus seeks a licensed Physical Therapist to provide coordinated home health care across age groups. You will plan, implement, and evaluate care plans, conduct PT treatment based on referrals, and collaborate with an interdisciplinary team to restore or maintain patient well-being.

The role involves patient/family education, transfer assistance, and adherence to safety and documentation standards in a home-health setting.

Qualifications

• Education/Licensing/Certification: B.S. in Physical Therapy from an APTA-approved program.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR, negative TB screen and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in acute care or rehab setting.
• Therapy skills as defined by generally accepted standards of practice.
• Good interpersonal skills.
• Knowledge of durable medical equipment.

Responsibilities

• Provides care while following infection control measures.
• Accepts clinical assignments within competence to care for patients.
• Monitors cases for third-party payor requirements.
• Communicates effectively with patient/family and team.
• Performs initial and ongoing clinical assessments and OASIS as appropriate.
• Prepares and revises Plan of Care with physician and care-team.
• Documents progress notes and orders; updates clinical records.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
• Assists patient/family with transfer techniques and safe use of equipment.
