Job summary

Providence at Home with Compassus is seeking a Physical Therapist to perform home-based therapy evaluations, develop plans, and implement treatment with the goal of improving function and discharge readiness. You will work within a multidisciplinary team, coordinating with physicians and staff to ensure high-quality care.

The role requires a CA license, a Bachelor's in Physical Therapy, and comfort in teaching patients and families to safely continue therapy at home.

Qualifications

• Bachelor's degree in Physical Therapy and active CA license.
• 1 year therapy with adults preferred.

Responsibilities

• Performs comprehensive patient assessments, including ROM, manual muscle testing, gait, and functional analysis.
• Plans and administers physician-ordered physical therapy programs to restore function and relieve pain.
• Records and evaluates assessment findings, plans, and progress notes for regulatory compliance.
• Administers therapeutic exercises and activities with equipment and devices.
• Provides treatments with heat/cold and other modalities as prescribed.
• Instructs patients and families on home exercise and safe equipment use.
• Coordinates with physicians and team to adjust treatment plans.
• Identifies referrals to additional disciplines to support goals.
• Supervises licensed PT aides and ensures care standards.
