Job summary

Providence at Home with Compassus is seeking a qualified Physical Therapist to provide in-home therapy evaluations, treatment planning, and discharge planning as part of a multidisciplinary team. The role emphasizes communicating client-related issues to staff and coordinating care across services.

You will supervise aides, orient staff, and support home-based rehabilitation goals. The position requires licensure, CPR/BLS, and the ability to travel.

Qualifications

• Required - Bachelor's Degree in Physical Therapy from an institution approved by the state board.
• Preferred - 1 year Therapy with adult physical disabilities.
• Current, unencumbered Physical Therapist license and CPR/BLS certification.

Responsibilities

• Perform comprehensive patient assessments to establish treatment plans.
• Plan and administer physician-ordered PT programs to restore function.
• Document assessments, treatment plans, and progress notes.
• Supervise PT aides and ensure care standards.
• Instruct patients and families on home exercise and device use.
• Collaborate with physicians and multidisciplinary team to coordinate care.
• Identify referrals to other disciplines as needed.
