Job summary

Providence at Home with Compassus seeks a Physical Therapist to conduct home-based evaluations, create and implement treatment plans, and discharge planning. You will work with a multidisciplinary team to communicate client needs and ensure high-quality care in the home setting.

Responsibilities include comprehensive assessments, administering PT programs, documenting findings, supervising licensed personnel, and coordinating with physicians to integrate therapy into the overall plan of care.

Qualifications

• Holds a Bachelors Degree in Physical Therapy from an institution approved by the state board.
• Current, unencumbered Physical Therapist license within 30 days of hire.
• BLS certification and AHA CPR as required.
• CA PT license upon hire (Vendor Managed).

Responsibilities

• Performs comprehensive patient assessments to establish treatment plans.
• Administers physician-ordered PT programs to restore function and reduce pain.
• Records assessment findings, treatment plans, and progress notes.
• Supervises PT assistants and aides to ensure quality care.
• Coordinates with physicians and team members to integrate therapy into care plans.
