Job summary

Providence at Home with Compassus seeks a licensed Physical Therapist to deliver coordinated home health care to patients of all ages. You will plan, implement, and evaluate plans of care, collaborate with the interdisciplinary team, and guide families in therapy and equipment use.

The role emphasizes OASIS assessments, patient safety, and adherence to care protocols within a weekday, day-shift schedule, in Portland-West OR Home Health. A current PT license and driver’s license are required.

Qualifications

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR, negative TB screen and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting.
• Knowledge of durable medical equipment.
• Good interpersonal skills.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain wellbeing.
• Performs initial and ongoing clinical assessments including OASIS as appropriate.
• Documents progress notes and orders; revises the Plan of Care with the physician.
• Provides treatment for neuromuscular, neurological, and orthopedic disorders per referral.
• Instructs patients and families on transfer techniques and safe equipment use.
• Communicates with the team and coordinates referrals to ensure continuity of care.
