Job summary

Providence at Home with Compassus is seeking a Physical Therapist to evaluate, plan, implement, and discharge patients in their home setting. You will work with a multidisciplinary team to ensure high-quality care and coordinate data for home-based treatment decisions.

The role requires a Bachelor's in Physical Therapy and current state licensure. Preference given to those with 1 year of adult therapy experience. Travel to patient homes is expected as part of the job.

Qualifications

• Bachelor's degree in Physical Therapy from an approved program.
• 1 year therapy experience with adults preferred.

Responsibilities

• Performs comprehensive patient assessments and establishes plans.
• Plans and administers PT programs to restore function and reduce disability.
• Records and evaluates findings to guide care and meet regulatory requirements.
• Administers exercises and activities with appropriate equipment.
• Instructs patients and families for safe home exercise and equipment use.
• Collaborates with physicians and team to coordinate care and adjust plans.
• Identifies referral needs and coordinates additional services.
• Supervises PT aides ensuring quality and compliance.
