Job summary

Providence at Home with Compassus seeks a licensed Physical Therapist Assistant to provide in-home therapy under the supervision of a Physical Therapist. You will implement treatment plans, document visits promptly, and communicate with the care team to support patient independence.

Responsibilities include assessing patient needs across ages, ensuring safety, coordinating with other disciplines, and scheduling visits while maintaining compliance with agency standards and payer requirements.

Qualifications

• Must have Associate degree in Physical Therapy and current PTA license.
• Preferred at least 1 year of PTA experience in home care.
• Current BLS certification preferred.

Responsibilities

• Deliver therapy services per the Plan of Care under the supervision of a PT.
• Document visits accurately the day of service.
• Maintain patient safety and adapt interventions for home settings.
• Coordinate with interdisciplinary care team and refer when needed.
• Schedule and manage home visits and related documentation.
