Job summary

Compassus Providence at Home with Compassus is seeking a Physical Therapist to perform comprehensive home health evaluations, develop and implement individualized treatment plans, and discharge planning in collaboration with the multi-disciplinary team.

You will supervise home health aides, travel to patient homes, and educate patients and families on exercises and safety. Requires state licensure and a commitment to high-quality patient care.

Qualifications

• Bachelor's Degree in Physical Therapy required from state-board approved institution.
• Preferred 1 year therapy with adult physical disabilities.
• Must have reliable transportation and ability to travel to patient homes.

Responsibilities

• Perform comprehensive patient assessments (ROM, manual muscle, gait, functional analysis).
• Plan and administer physician-ordered physical therapy programs to restore function and reduce pain.
• Record and evaluate assessment results and progress notes to guide care.
• Supervise licensed PT assistants and aides; orient staff as needed.
• Coordinate with physicians and interdisciplinary team to adjust treatment plans.
