Job summary

Matrix Medical Network seeks a dedicated Nurse Practitioner to perform Comprehensive Health Assessments in members’ homes or SNFs. Visits run up to one hour and focus on health, history, medication adherence, and risks to close care gaps.

Providers collaborate with physicians, case managers, and staff, delivering education and follow-up while enjoying flexible scheduling and potential FT opportunities.

Qualifications

• Master’s Degree or commensurate experience.
• Current NP licensure in state of practice.
• AANP, AACN, or ANCC board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license.
• Auto liability insurance coverage as required by state.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment.
• Order and administer point-of-care and lab tests as needed.
• Coordinate with physicians, case managers and family members.
• Collaborate with PCP on patient education and follow-up.
• Provide services at members’ residences (homes or SNFs).
