Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes, reviewing health history, medications, and risks to gain insight into overall health. Visits last up to one hour; you’ll coordinate with physicians and care teams, provide education, referrals, and documentation; travel to member residences including skilled nursing facilities.

We offer flexible scheduling, competitive pay based on assessments completed, sign-on bonus of $1,500,

Qualifications

• Master’s degree or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain it.

Responsabilités

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation according to Matrix guidelines and protocols.
• Administer and order point of care and lab-based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary.
• Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities

Formation

Master’s Degree or commensurate experience
