Job summary

Matrix Medical Network is seeking Nurse Practitioners to conduct Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities across the United States. Each visit lasts up to an hour and focuses on health history, medications, social factors, and risk assessment to close care gaps.

Qualified candidates hold an NP license in the practice state, board certification, and at least 1 year of NP experience; strong communication and computer skills are required.

Qualifications

• Master’s degree in nursing and NP licensure; board certification in Adult/Gerontology or related field.
• Current NP licensure in state of practice with prescriptive authority as needed.
• CPR/BLS/ACLS certification.
• 1 year NP experience preferred; new grads invited.
• Valid driver’s license; auto insurance or access to state-appropriate coverage.

Responsibilities

• Perform comprehensive in-home health assessments lasting up to one hour.
• Review medical history, medications, social factors, and risks to close care gaps.
• Administer and order point-of-care tests and lab work as needed.
• Coordinate with PCPs, case managers, and family members for follow-up and care planning.
• Provide services at members' homes or SNFs as appropriate.
