Job summary

Matrix Medical Network seeks Advanced Practice Providers to perform comprehensive in-home health assessments. Each visit lasts up to one hour, evaluating medical history, medications, social context and health risks to close care gaps.

You will order point-of-care tests, collaborate with PCPs and care teams, and deliver care at patients' homes or SNFs. Flexible scheduling and competitive compensation available.

Qualifications

• Master’s degree or commensurate NP experience and licensure.
• Current NP licensure in state of practice and prescriptive authority as needed.
• Board certification in Adult, Gerontology or Family NP specialties (AANP/AACN/ANCC).
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and automobile insurance coverage.
• Strong computer skills and familiarity with EMR/health-record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate with PCP on education and follow-up care.
• Provide services at patients’ homes or Skilled Nursing Facilities.
