Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes or Skilled Nursing Facilities. Each visit lasts up to one hour, reviewing medical history, medications, social context, and risks to close care gaps.

We offer flexible scheduling (including weekends) with pay based on assessments completed, mileage reimbursement, and retirement benefits. Education and licensure support are provided to help you advance within the network.

Qualifications

• Must hold NP licensure in state of practice.
• Current board certification in relevant specialty.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred.
• Valid driver’s license.
• Automobile insurance coverage for state of residence (or ability to obtain).

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis/treatment, health education, and referrals with clear documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other care team.
• Coordinate with Primary Care Physician for patient education and follow-up.
• Provide services at the member’s home or Skilled Nursing Facilities as appropriate.
