Job summary

Matrix Medical Network is seeking experienced Nurse Practitioners to perform in-home Comprehensive Health Assessments. You will review medical history, medications, and risk factors to identify care gaps and improve health outcomes.

You’ll order tests, coordinate with physicians and case managers, and deliver clear documentation with patient education. Providers work in members’ homes or facilities, with flexible scheduling Sunday–Saturday and opportunities to transition to full-time roles.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice
• AANP/AACN/ANCC board certification in relevant NP specialty
• Current BLS/ACLS or CPR certification
• 1 year NP experience preferred
• Valid state driver’s license
• Auto insurance coverage as required by state
• Strong computer and health records software knowledge
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment plans
• Administer and order point of care and lab-based testing
• Collaborate with PCP, case managers and other providers
• Educate patients and provide follow-up instructions
• Provide services at members’ homes or other approved sites
