Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home Comprehensive Health Assessments. Each visit lasts up to one hour and evaluates medical history, medications, adherence, and social factors to identify care gaps and tailor interventions.

Providers collaborate with physicians and care teams, order point-of-care testing, and deliver care at the patient’s home or in Skilled Nursing Facilities. Flexible scheduling and competitive pay reflect your availability.

Qualifications

• Master’s degree in nursing or commensurate experience.
• Current NP licensure and prescribing authority as needed.
• Board certification in relevant NP specialty.
• BLS/ACLS certification and valid driver’s license.

Responsibilities

• Conduct adult/geriatric in-home assessments including history and diagnosis.
• Administer and order point-of-care and lab tests as needed.
• Coordinate with physicians, case managers, social workers, and caregivers.
• Collaborate with PCP for education and follow-up.
• Provide services at the member’s home or in SNF settings.
