Job summary

Matrix Medical Network in the United States seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or SNFs. Each visit lasts up to one hour and includes review of medical history, medications, social factors, and risk assessment to identify care gaps and improve outcomes.

We offer flexible scheduling (including weekend hours), competitive pay based on the number of assessments, mileage reimbursement, 401(k), and ongoing education.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in relevant NP specialty.
• Current BLS/ACLS certification.
• Active driver’s license and auto insurance.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis.
• Provide education, referrals, follow-up and documentation per Matrix guidelines.
• Order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and other care team members.
• Deliver services at the member’s home or Skilled Nursing Facility.
