Job summary

Matrix Medical Network seeks experienced NP providers to conduct Comprehensive Health Assessments in members' homes nationwide. Visits last up to one hour, focusing on medical history, adherence, and social determinants to close care gaps.

You’ll collaborate with physicians and care teams, with support for licensure expansion and continuing education. Flexibility is emphasized with scheduling across week days and weekends; competitive pay per assessment and monthly earnings potential from 1,800

Qualifications

• NP licensure in practicing state and prescriptive authority if needed.
• Board certification in adult/gerontology/family or emergency specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance for residence state.
• Strong computer and EMR software proficiency.
• Excellent verbal and written communication skills.

Responsibilities

• Perform comprehensive adult/geriatric health assessments in home settings.
• Document findings clearly and provide follow-up referrals as needed.
• Coordinate with physicians, case managers and care teams.
• Collaborate with PCP for patient education and care plan.
• Deliver services at members' residences or SNFs.
