Job summary

Matrix Medical Network, a national provider of in-home clinical assessments, is seeking Nurse Practitioners to perform Comprehensive Health Assessments. Each visit lasts up to an hour and covers medical history, medication adherence, and social determinants of health to close care gaps.

Providers collaborate with PCPs and care teams, offering services at members' homes or skilled nursing facilities. Flexible scheduling and competitive compensation are available, with opportunities for licensure

Qualifications

• Must hold NP licensure in state of practice with prescriptive authority or ability to obtain
• AANP, AACN, or ANCC board certification in Adult/Adult Gerontology/Family/ Emergency- or Acute Care
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred; new grads welcomed
• Valid driver’s license for state of residence and auto insurance coverage
• Strong computer skills and familiarity with health records software

Responsibilities

• Review member health, medical history, medications, social environment and other risks during visits lasting up to one hour
• Perform adult/geriatric assessments including medical history, diagnoses, treatment, education, referrals, and follow-up per Matrix guidelines
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family, and other care team members
• Provide services at members' residences, including homes or skilled nursing facilities
